Description
Responsible for the overall administration of patient services for patient care program, delegating responsibility as appropriate to program coordinators.
Reports to Executive Director.
QUALIFICATIONS
* A registered nurse or hospice director experience preferred.
* Minimum of two years nursing or experience in a home health/hospice or public health setting.
* Minimum 3 years experience in supervisory or management position.
* Demonstrated ability to manage all aspects of terminal care with expertise in pain and symptom management.
* Demonstrated skill in administrative performance, including expertise in both written and oral communication, supervision of staff, documentation requirements, cost efficiencies, marketing referral development, service coordination and case management.
* Demonstrated ability to execute an effective, common sense, supportive, sensitive approach in response to needs of staff and patient/family.
* Demonstrated administrative ability including knowledge of health care policies and program responsibilities.
* Experience in development of administrative policies and procedures and design of programs for implementation and compliance.
FUNCTIONS
Marketing & Admissions
* Proactively identifies hospice clinical programs and specialized services to uniquely position Valor HospiceCare in the market.
* Responsible with Customer Development Manager for meeting or exceeding Valor HospiceCare’s targeted goals for ADC growth across all sites. Participates in design and execution of Valor HospiceCare marketing plans.
* Participates in professional relations, outreach and education to enhance Valor HospiceCare recognition and reputation within the medical communities it serves and to optimize patient referrals across all sites.
* Assists Customer Development Manager with supporting Medical Director marketing and referral activities.
* Is Admissions RN for marketing team in Tucson service area.
Operations
* Oversee Tucson team markets for clinical care, and assisting with census development goals.
* Participate in overall agency planning, including policies, procedures and guidelines to fulfill program objectives.
* Plan and implement specific clinical and administrative projects, coordinating them with overall agency plans; Identify need for changes in program and services, develop proposals for consideration by Program Director and other members or Leadership Team, as appropriate.
* Monitor agency compliance with state health department/federal government standards and regulations for licensure and hospice certification.
* Provide guidance in communication, coordination, and effective operations relating to patient care through development and utilization of recording and reporting systems, manuals and other resource materials.
* Oversee the maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.
* Oversee Pharmacy, Medical Supplies, Durable Medical Equipment, Therapies programs, including compliance with regulations and cost containment initiatives.
* With Medical Director, oversees delivery of patient services, budgets and cost containment to plan and compliance with agency policies.
* In addition to the Routine Level of Care, ensures that the Continuous Care and Inpatient/Respite Programs are meeting or exceeding policies and procedures and established program guidelines.
* Proactively addresses problems associated with the delivery of clinical services, staff development needs, to put forth anticipate suggestions for appropriate additions/changes in policies and procedures, and to give input to enhance and improve the middle management function.
* Assist in the development of personnel policies and monitor compliance in areas of responsibility.
* Identify need for staff education and assist in the development of orientation and in-service programs for interdisciplinary team for compliance with Medicare regulations for palliative care training and in-services.
* Direct plans for recruitment and career development of patient care team personnel.
* With Human Resources guidance, provide information for inclusion in written performance evaluations for all staff assigned to team in cooperation with appropriate members of the Governing Body.
* Supervise and coordinate utilization review activities.
* Provide input to related administrative/computerized systems and monitor the processing of data relating to patient care.
* Provide supervision of those clerical staff supporting Clinical Care team, including PatientCare Associates for Tucson office.
* Perform specific assignments as directed by the President & Administrator, Program Director or designee.
Nursing and Aide Services
* Provide leadership and counsel to nursing and aide personnel to measurably enhance staff development.
* Serve as Director of Nursing, and manages PatientCare Coordinators (PCC) in Tucson to assure compliance with agency policies and procedures.
* Participate and provide leadership in the team for regular interdisciplinary team meetings; the development, review and revision of the patient/family plan of care; facilitation of problem solving sessions; and delivery of team support.
* Oversee through PCCs, clinical supervision of nursing personnel, home health aides and homemakers, and conduct written performance evaluations.
* Supports on-call initiatives of nursing team.
* Ensures staffing requirements for growth and manage costs for labor services to budget and within company and Hospice guidelines
* Ensures patient care costs and related costs are within budget.
* Conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members.
* Conducts patient care, if applicable.
* Oversee palliative care program for pre-hospice evaluations and hospice consultations; will develop plan and implement palliative care program for medical group coverage for Medicare Plan B for hospice and non-hospice eligible programs.
Social Services
* Working with the Program Director and PCC, ensures interdisciplinary team approach to holistic care for the patient, caregiver and families.
* Oversees management of all Social Services functions.
